Advance Care Planning can help prepare for a time when you may not be able to make health or personal care decisions for yourself and when someone else, your Substitute Decision Maker, would make decisions for you.
In this workshop, we follow a family’s journey through health care situations to learn about:
– What is Advance Care Planning
– Who is your Substitute Decision Maker and how do they make decisions
– Exploring values and wishes
– Health care consent
